Fausses couches à répétition est définie comme la perte de trois ou plusieurs grossesses. Fausses couches à répétition est un état hétérogène qui a de nombreuses causes possibles; plus d'un facteur contributif peut expliquer les pertes de grossesse récurrentes. Quand un patient subit des pertes de plusieurs de grossesse, par conséquent, il sera judicieux d'évaluer la patiente à l'égard des scenaria possibles suivantes:

1 - Les facteurs génétiques

Tous les couples ayant des antécédents de fausses couches à répétition devraient avoir le caryotype du sang périphérique effectuée. La constatation d'une anomalie du caryotype parental devrait inciter renvoi à un généticien clinique.

2 - La faiblesse du col

C'est une cause anatomique connue de la perte de fausse couche et la grossesse peut être récurrent. Le col (ou col de l'utérus) est normalement assez fort pour demeurer fermées pendant toute la durée de la grossesse, en dépit de la taille sans cesse croissante et le poids du contenu utérus. En fait, même à terme, les changements hormonaux doivent avoir lieu selon lequel les hormones spécifiques sont libérés pour agir sur le col, ce qui rend plus souple et plus mince et en lui permettant de se dilater dans le travail. Avec une béance du col, le col est anatomiquement faible, provoquant l'utérus à perdre de son contenu - vers la mi-grossesse. Typiquement, une fausse couche provoquée par une béance du col survient entre seize et vingt-quatre semaines de gestation.

3 - Facteurs endocriniens

Le facteur hormonal endocrinien terme dans l'infertilité se réfère principalement à l'absence d'ovulation, ce qui entraîne des troubles du cycle menstruel (oligoménorrhée, aménorrhée primaire ou secondaire) et, moins fréquemment à l'état de follicule unraptured lutéinisé. En cas de oligoménorrhée ou aménorrhée résultats satisfaisants peuvent être obtenus avec les méthodes classiques de l'induction d'ovulation, en évitant l'utilisation de techniques de reproduction assistée.

4 - les facteurs immunitaires

5 - agents infectieux
TORCH (toxoplasmose, autre [syphilis congénitale et virus], rubéole, cytomégalovirus
et de l'herpès simplex virus) le dépistage est inutile dans l'enquête sur récurrentes
fausse couche.

6 - Les défauts hérités thrombophiliques

Héritée défauts thrombophiliques, y compris la protéine de résistance C activée (le plus souvent à cause de facteur V Leiden mutation génétique), les carences de la protéine C / S et l'antithrombine III, hyperhomocystéinémie et la mutation du gène de la prothrombine, sont établies les causes de la thrombose systémique.

Si vous avez éprouvé fausses couches à répétition, avec ou sans traitement de FIV, il est utile d'examiner la cause avant de tenter un autre traitement de FIV. Parfois, une erreur peut être évitée en prenant de petites mesures, il est donc logique d'identifier la cause avant de faire une tentative de FIV.

Médicaments pour le FIV

Il est nécessaire de prendre certains médicaments pendant le cycle de FIV en vue de préparer le corps pour le traitement. Les instructions pour chaque médicament varient de patient à patient. Nos spécialistes au Nord de Chypre FIV analysera votre dossier de près afin de déterminer quels médicaments à utiliser, ce qui la dose à prendre, quand à administrer les médicaments et combien de temps pour les prendre.

Agonistes de la GnRH - Lucrin, pris comme une injection juste en dessous de la peau et Synarel, un spray nasal, permettent au corps de produire un plus grand nombre d'œufs de haute qualité pendant le cycle de traitement. Ils empêchent également la montée au milieu du cycle hormonal qui peut entraîner l'annulation d'un cycle.

Antagonistes - Antagon et Cetrotide sont des antagonistes de la gonadolibérine (GnRH) et sont utilisés pour prévenir l'ovulation prématurée. Ces médicaments sont administrés par injection et la durée du traitement est généralement de trois ou quatre jours.

Gonadotrophines - Les gonadotrophines sont prises comme des injections sous-cutanées qui fournissent la stimulation des follicules qui contiennent les œufs pendant la phase de stimulation. Gonal F, Bravelle, Follistim, Pergonal et Repronex sont les gonadotrophines les plus couramment utilisés.

hCG - hCGs sont pris comme une injection intramusculaire et sont utilisés pour induire la maturation finale des changements dans les oeufs et les préparer à la récupération. Le hCGs les plus couramment prescrits sont Pregnyl, Ovitrelle Choriomon.

Doxycycline - doxycycline, un antibiotique administré sous forme de pilule, est donnée au partenaire masculin au cours du cycle de stimulation de la femme afin de réduire les faibles niveaux de bactéries qui peuvent être trouvés dans le sperme et qui peut compromettre la performance du sperme au cours d'un cycle de FIV. Il est également accordée à la partenaire féminine pour réduire le risque d'infection après l'aspiration des follicules au moment de la récupération des œufs.

Progestérone - progestérone naturelle peut être pris comme une injection intramusculaire quotidienne commençant deux jours après le prélèvement des ovules et continue jusqu'à ce que le placenta est fait des quantités suffisantes de progestérone. La progestérone peut également être donné sous la forme d'un gel vaginal ou de suppositoires vaginaux ou de comprimés (Prometrium).

Enquête infertilité

L'infertilité est souvent définie comme l'incapacité de concevoir naturellement après avoir essayé pendant un an sans contraception. Les femmes qui sont en mesure de tomber enceinte, mais qui sont incapables de mener la grossesse à terme sont également considérés comme stériles. Environ 10% de la population américaine, et environ 9% de la population européenne entre les âges de 15-44 ont soit des difficultés concevoir ou à mener une grossesse à terme.

Même si l'infertilité est souvent perçue comme un problème de femmes en raison de son lien direct avec la grossesse, l'infertilité féminine et masculine ont environ le même niveau de prévalence au sein de la société. Environ un tiers des causes de l'infertilité sont associés à l'infertilité féminine et un autre tiers est associée à l'infertilité masculine alors que le reste est soit en raison d'un combiné masculin-féminin facteur ou une infertilité inexpliquée.

Si vous avez essayé de tomber enceinte pendant l'année écoulée et n'ont pas été en mesure de tomber enceinte, alors vous pourriez appartenir à l'un de ces groupes. Pour les partenaires qui n'ont pas réussi à obtenir une grossesse pendant une année entière, une série de tests est en ordre. Enquête infertilité fonctionne d'une manière double où les deux partenaires subissent un processus de test. Le point de départ de tests d'infertilité seraient les suivantes:

- Pour les partenaires masculins, une analyse du sperme est nécessaire. L'analyse du sperme Krueger
fournit des informations sur le nombre de spermatozoïdes, le volume, la motilité, la morphologie et
quelques facteurs plus encore. Ce serait le point de départ de l'enquête infertilité.

- Pour partenaire de sexe féminin, les tests hormonaux est en ordre. Si le partenaire de sexe féminin a
menstruations régulières, puis des tests d'hormones telles que la FSH, LH, estradiol,
Prolactine et la TSH. Si la femme est jeune, mais n'a pas régulier
périodes menstruelles, puis mis à part les tests hormonaux, une enquête plus approfondie
peut être nécessaire de voir dans la fonction ovarienne et de l'utérus. Irrégulier
périodes pourrait être due à syndrome des ovaires poly kystiques (SOPK) ou tout autre
problèmes.

Lorsque les tests hormonaux standard et enquêtes sonagramme ne parviennent pas à fournir une réponse, processus de tests plus invasifs seront dans l'ordre. Partenaire de sexe féminin pourrait être nécessaire d'avoir une hystéroscopie ainsi que certains tests de maladies infectieuses telles que la chlamydia, IgG CMV et la toxoplasmose.

Lorsque tous les processus de test ci-dessus ne parviennent pas à produire et à répondre, puis thrombophilie défauts et les problèmes du système immunitaire sont à étudier. Surtout pour les femmes ayant des fausses couches répétées, la thrombophilie est l'un des principaux suspects.

Parfois, les patients qui terminent leur première série de tests peuvent être proposés des traitements de fertilité avant d'essayer les tests plus approfondis et envahissantes. C'est tout simplement parce que, la probabilité d'appartenir au groupe infertilité inexpliquée est très élevé et les procédures de tests coûteux peut être très accablante. Par conséquent, avant de tenter ces tests, un traitement de FIV peut être tentée en premier et si le résultat est négatif, d'autres tests peuvent être effectués. Dans la plupart des cas, les partenaires sont en mesure d'obtenir une grossesse sans consulter les procédures de test envahissantes.

Surrogacy

Our US-based surrogacy program works in a dual phase. The legal work, surrogate recruitment, child delivery and court proceedings all take place in the US while the actual IVF treatment takes place in Cyprus. Cyprus does not have definte laws for surrogacy, therefore, all the legal work involved needs to be done in the US bascked by the US legal system in order to avoid future problems. The fact that your IVF treatment takes place in Cyprus is something that drives the cost down and in fact offers a higher chance of success.

The first step in surrogacy IVF treatment, once the necessary framework has been established, is to design a contract which specifies the rules and the steps involved in thesurrogacy arrangement. Our lawyer in the US is specialized in surrogacy cases and has a very good reputation for it. We will make the necessary legal arrangements, prepare the court proceedings for you to be the legal parents of the child. For a contract to be ready, we will be needing to match you with a surrogate and have everything discussed thoroughly so both parties can agree on common grounds.

Once we have the legal arrangements sorted out, the next step would be to get all your physical features, blood types and any other physical features you would like to specify for the donor matching process if you will be needing an egg donor. Once we find a suitable donor in our database, who fits your physical profile, matches with respect to blood types, and has her menstrual periods around the same dates as the surrogate, we will proceed with the medication stage. In this stage, your donor will receive certain hormone supplements in order to stimulate the egg count and quality. At the same time, the surrogate will receive certain medication in order to have her endometrium ready for a successful pregnancy. If your own egg reserves and egg quality allow us to use your own eggs, then you will not be needing a donor.

Once the donor (or you) and the surrogate are simultaneously prepared, we will have the surrogate fly out to Cyprus for the IVF treatment. At this point, we will perform oocyte retrieval from you or your donor, fertilize with your husband's sperm and create the embryos. The highest quality embryos, in turn will be transferred into the surrogate's uterus on day 3 of fertilization. The surrogate will go back to the US and the treatment will be over. Once the baby is born after nine months, you will make another trip to the US to hold your child for the first time. We are able to transfer up to 3 embryos in a given transfer cycle.

Our main aim is to provide a low cost treatment without sacrificing the quality and reliability. Therefore, we make sure the surrogate is bound by a very well-prepared contract by someone who has significant amount of experience in this field. The actual treatment, is carried out by us. Our success rates are well above world averages, especially in egg donation cases. The reason is, Cyprus is a small country and we tend to contact the donors who have donated before, who have good quality eggs and resulted in a successful pregnancy, thus maximizing the chances of pregnancy, which explains our 81% success rate in 2011, in egg donor IVF cases.

Microsort IVF and PGD

Microsort IVF (invitro fertilization) with PGD (pre-genetic diagnosis) is considered to be the most effective and accurate gender selection method available.

The treatment process and medication is the same as Microsort with IVF where the sperm is sorted into X and Y, however once the embryos are formed and have developped to the 8 cell stage, there is a biopsy perfomed for both genetic testing and gender (sex) selection.

Combining the PGD with Microsort increases the success of the desired gender from 75-90% up to 99.9% and allows for genetic testing to take place to ensure that the healthiest embryos are transfered.

PGD is a procedure that is generally recommended for older women who’s eggs have a higher risk of abnormalities.

Microsort IVF

Microsort is the process of sorting out the male sperm into X and Y before the ICSI takes place.  The treatment resembles a regular cycle of IVF however the Microsort sperm is used for fertilizing the eggs that are retrieved. 

Depending on the development of the follicles, the egg retrieval is done and sperm is collected from both partners on the same day.

The sperm is then sorted at the Microsort facitlity and the sorted sperm is injected with ICSI into the egg. The eggs are then fertilized and incubated for development.

The embryo transfer is then performed.

The medication and dosage used is the same as a regular IVF cycle and will vary from patient to  patient.

Gender Selection PGD

Pre-Implantation Genetic Diagnosis (PGD) is the technology used to improve the success of pregnancy and birth. Couples who have had repeated miscarraiges or unsuccesful IVF cycles as well as couples who might be at risk at passing inherited genetic diseases should consider having this testing done along with IVF.

PGD is also a very successful method to choose the gender of your child and is performed during the IVF cycle after multiple eggs are produced, collected and fertilized. Once the embryo has been formed, one to two cells are retrieved and an embryo biopsy is performed on it.  The cells are analyzed at our in house PGD Laboratory  to establish which embryos are healthy and free of genetic abnormalities. This testing which is sophisticated and advanced helps to ensure the baby is healthy.  This process helps to select both healthy embryos along with determining the sex of the baby.

After the PGD is performed, the chances of achiving the desired sex is 99.9%.

The PGD process is the same as regular IVF however the embryos are kept in the laboratory for a 3-4 days longer to undergo biopsy. The following chromosomes are biopsied for abnormalities: X, Y, 13, 18 and 21.

Egg Donation Program

Egg Donation IVF is the treatment method for women who do not have healthy eggs for conception. The following list indicates women who are ideal candidates for egg donor IVF treatment at North Cyprus IVF clinic:

- Women who are going through premature menopause
- Women who have had their ovaries removed and therefore do not produce eggs
- Women who have undergone cancer treatment and have damaged ovaries
- Women who were born without functioning ovaries (Turner's syndrome)
- Women with poor ovartian reserves
- Women with hereditary genetic disorders
- Women who have had multiple miscarriages due to chromosomal abnormalities

Who Are Egg Donors at North Cyprus IVF?

Our egg donor base is consisted predominantly of international students studying at various universities in Cyprus. As a result, majority of our egg donors are between 20-24 years of age. This is precisely why our success rates are considerably higher than most of the IVF clinics in the world. Our egg donors are very young and most of them have previous positive donation histories.

In general, our egg donor selection criteria can be summarized as follows:
- women between 20-29 years of age.

Microsort IUI

IUI (intrauterine insemination) treatment is generally recommended for patients in their 20’s who prefer a non-invasive alternative to IVF.

A mild dose of medication is prescribed for ovulation in order to increase the number of eggs that are produced. The male sperm is then sorted into X and Y with the Microsort procedure and inserted into the uterus once the eggs have matured and are ready for fertiization.

This method is 90% accurate for female and 75% accurate for male offspring.

As the eggs are not retrieved from the female, there is no biopsy or genetic testing performed to ensure 100% accuracy.

The success rates for IUI are about 5-30% depending on the age of the female and multiple tries might be required until pregnancy is achieved.

IVF with ICSI - North Cyprus IVF

IVF which is the common term for In Vitro Fertilization, is the process of creating an embryo using the egg and sperm in a laboratory environment. Once the embryos have been formed, the embryo transfer is physically placed in the uterus.

Here is the step by step process of the procedure:

Before embarking on the IVF procedure, we ask that you have some initial hormone test results for evaluation. These tests are FSH, LH, E2 and Prolactin and are normally done between day 2-4 of the menstrual cycle.  An alternative test would be the AMH which can be done on any day of the cycle.

Once you are considered a qualified candidate, the process can begin.

The first step is to begin with the ovulation induction with the assistance of fertility medications to control the timing and mutliple growth of the egg. The medication regimen will depend on each individual case and will be prescribed accordingly.

In order to shorten the stay in Cyprus, we monitor your medication intake to ensure good development of the follicles. Beginning the medication protocol in your home country will help keep you visit in Cyprus to 6-7 nights.

Egg retrieval is done upon arrival in Cyprus with the assistance of ultrasound imaging that guides the hollow needle through the pelvic cavity. In order to increase your comfort, light sedation and local anesthesia are used during this process. The eggs are removed by follicular aspiration.

At the same time that the eggs are collected, the sperm is also collected from the male partner. This is generally done via ejaculation but if there are any problems with it there are procedures that can be used to extract the sperm manually.

Insemination is the name of the process where the eggs and sperm are placed in incubators for the fertiliztion to occur. Most times however, ICSI (intracytoplasmic sperm injection) is used for greater accuracy of fertilization.

The fertilized eggs are then monitored for 3 days until they have grown enough for the embryo transfer.

During the final stage of the process which is the embryo transfer, a speculum is inserted into the vagina in order to expose the cervix. 2-3 embryos are then suspended in fluid and gently placed in the womb through a catheter. This process is also guided by ultrasound in order to confirm accuracy of embryo placement. 10-12 days after the embryo transfer the patient is asked to undergo a blood test in order to find out if the fertilization process was successful.

After Care

Once your embryo transfer has been completed, you are free to travel back to your home country. You can travel the day of the transfer or the following day as the result will not be effected.

Before you leave our clinic, we will offer you a prescription for further medication which you will need to use for approximately 12 weeks in the event of a positive pregnancy result.

If you opt not to receive the prescription it is imperative that your GP is able to provide you with this.

After a confirmed pregnancy, you will continue with your local GP to track and monitor the progress.

Vitamins and IVF

Vitamins are an essential part of your treatment. It has been shown that folic acid supplements not only help you maintain a healthy pregnanyc, but also help with your baby’s neural development, therefore, it is important to start using folic acid prior to embryo transfer and continue using for the duration of your pregnancy.

Similarly, use of Vitamin C tablets (precisely, 500 mg) has been identified with increased quality of eggs retrieved, therefore, it will be important to use vitamin C supplements daily for at least 2-3 weeks prior to egg retrieval.

Finally, because your daily food intake may not provide you with all the essential nutrients, it will be important to use a prenatal vitamin complex prior to and throughout your pregnancy. Pregnacare or any other one-a-day prenatal vitamin complex will be useful.

Where to get your scans?

Since majority of our patients come from abroad, and given that in our hectic lives, we all have work/family or other commitments, not everyone is able to travel for treatment for a long period of time. In order to shorten the length of your stay without compromising your chances of success, our protocols are prepared so that you can have access to your medication and follow a certain treatment calendar prior to your arrival in Cyprus. This minimizes your stay down to 8 days. However, in the meanwhile, while you are using your medication, you will need to have a couple of scans in order to observe how you are responding to the medication and whether we need any dose adjustments or additional supplements. For this end, you can either see your own gynecologist for the scans if they are willing to cooperate, or use one of the locations we recommend.

1- For UK patients, Baby Bond is a great place for the scans. The scans are reasonably priced and you are able to get last minute appointments. 2- For US patients, we have a number of locations that we can recommend. However, since the US has a very large geaography, our locations may need air travel, which may not be convenient to all patients. Please ask Ahmet directly about the US locations.

Par où commencer?

Voyager à Chypre

Notre clinique est située à Nicosie, Chypre du Nord. La aéroport le plus proche de notre clinique est de l'aéroport d'Ercan. Tous les vols à destination de l'aéroport d'Ercan ne passent par l'une des villes en Turquie, il n'y a pas de vols directs. La route la plus directe dans Ercan serait de voler à travers Istanbul.

Une autre alternative est l'aéroport de Larnaca dans le sud. Il est situé à environ 60 km au sud de notre clinique, et prend environ une heure pour parcourir. Parfois, les vols à destination de Larnaca peut être beaucoup moins cher en fonction de la saison, donc vérifier les deux aéroports prendra tout son sens avant de réserver vos vols.

Twins!

Hi Ahmet.

Just to let you know, I gave birth to twin boys, Ethan and Blake, on Friday 6th june.

Me and babies doing well and are all at home. Blake weighed 7lbs and Ethan weighed 6lbs 10oz.

I wanted to let you know as to say thankyou for everything you have done, and also could you please

Repeated IVF Failures

IVF failures are one of the most depressing experiences a couple can go through in their IVF journey. Having experienced several failed attempts can be very depressing and frustrating, especially when there is no explanation provided for them. At North Cyprus IVF, we make sure our patients are thoroughly analyzed before attempting another IVF cycle so that some known possibilities can be ruled out. Once a couple go through the standard testing procedures and these tests are inconclusive of the factors of infertility, then additional factors need to be taken into consideration. Some of the possibilities which need to be taken into consideration are as follows:

Uterine infection
The inside of the uterine cavity is normally considered to be a sterile environment. It has been strongly suspected that infection of the uterine cavity with bacteria may cause an inhospitable environment that would lead to failure of embryos to implant. In cases where there is a reason to suspect a uterine infection, an endometrial biopsy procedure may be called for in order to make an effective assessment. Normally, infections can be treated with antibiotic use. There are also other infectious diseases that can be considered during this investigation such as CMV IgG, Chlamydia and more.

Immune factors as a cause for IVF implantation failure
The immune system is designed to protect individuals from infection with microorganisms and to fight off abnormal processes in the body like cancer. In the areas of IVF, infertility and recurrent pregnancy loss, there has been an extensive amount of investigation as to the role of immune factors in the implantation process of the embryos. In some cases, the immune system is believed to see the embryo as a foreign object and reject its implantation.

Thrombophilias
Thrombophilias are conditions that result in an increased chance for blood clotting. There have been many studies showing an association between the presence of thrombophilias in women and the risk for miscarriage. Some studies also indicate that thrombophilias can also be associated with implantation failures. Certain genetic tests help identify the existence of thrombophilias in the female partner.

Infertility Investigation

Infertility is often defined as inability to conceive naturally after trying for one year without any contraception. Women who are able to get pregnant but who are unable to carry pregnancy to full term are also considered to be infertile. About 10% of the US population, and around 9% of the European population between the ages of 15-44 have either difficulty conceiving or carrying pregnancy to full term.

Even though infertility is often perceived as a women's problem due to its direct link to pregnancy, female and male infertility have about the same level of prevalence within the society. About one-third of the causes of infertility are associated with female factor infertility and another one-third is associated with male factor infertility when the rest is either due to a combined male-female factor or unexplained infertility.

If you have been trying to get pregnant for the past one year and have not been able to get pregnant, then you might belong in either one of these groups. For partners who have not been successful at achieving pregnancy for a full year, a series of tests is in order. Infertility investigation works in a dual manner where both partners undergo a testing process. The starting point of infertility testing would be as follows:

- For male partner, a semen analysis is required. Krueger's semen analysis

provides information on sperm count, volume, motility, morphology and

a few more factors. This would be the starting point of infertility investigation.

- For female partner, hormone testing is in order. If the female partner has

regular menstrual periods, then hormone tests such as FSH, LH, Estradiol,

Prolactine and TSH. If the female partner is young, but does not have regular

menstrual periods, then apart from the hormone tests, further investigation

may be necessary to see into the ovarian function and the uterus. Irregular

periods could be due to Poly Cystic Ovarian Syndrome (PCOS) or any other

problems.

When standard hormone testing and sonogram investigations fail to provide an answer, more invasive testing process will be in order. Female partner might be required to have a hysteroscopy as well as certain infectious disease tests such as Chlamydia, CMV IgG and Toxoplasmosis.

When all the above testing processes fail to yield and answer, then thrombophilia defects and immune system problems are to be investigated. Especially for women with repeated miscarriages, thrombophilia is one of the main suspects.

Sometimes, patients who complete their first round of testing can be offered fertility treatment before attempting the more in-depth and invasive tests. This is simply because, the likelihood of belonging in the unexplained infertility group is very high and the expensive testing procedures can be very overwhelming. Therefore, before attempting these tests, an IVF treatment can be attempted first and if the result is negative, then further tests can be performed. In most cases, partners are able to achieve pregnancy without consulting the invasive testing processes.
