Minimal Stimulation IVF - (Mini IVF) Treatment

Mini IVF is very similar to typical IVF in terms of procedures used during treatment. The main difference is that; while typical IVF aims to produce a certain amount off eggs for retrieval, mini IVF aims to produce only a few eggs of higher quality. In other words, mini IVF is performed to get a low count but higher quality of eggs through comparatively  weaker and/or lower doses of medications.

At North Cyprus IVF Centre this option of treatment is generally offered to patients; in their late 30s or early 40s who do not respond well to IVF stimulation protocols, has diminished ovarian reserves, are not willing to use high dose of IVF stimulation medication, whose oocyte quality has been compromised and FSH levels are high.

By the way, it should be noted that when women reach at a certain age, they face with depleted ovarian reserves as well as compromised quality. Therefore, given the fact that Mini IVF will only help to get slightly higher quality eggs, North Cyprus IVF center also recommends "IVF with Egg Donation" as a treatment option if getting a successful outcome is more important than using your own eggs.

If you need any further assistance, please feel free to get in touch with us.

Gender Selection PGD

Pre-Implantation Genetic Diagnosis (PGD) is the technology used to improve the success of pregnancy and birth. Couples who have had repeated miscarraiges or unsuccesful IVF cycles as well as couples who might be at risk at passing inherited genetic diseases should consider having this testing done along with IVF.

PGD is also a very successful method to choose the gender of your child and is performed during the IVF cycle after multiple eggs are produced, collected and fertilized. Once the embryo has been formed, one to two cells are retrieved and an embryo biopsy is performed on it.  The cells are analyzed at our in house PGD Laboratory  to establish which embryos are healthy and free of genetic abnormalities. This testing which is sophisticated and advanced helps to ensure the baby is healthy.  This process helps to select both healthy embryos along with determining the sex of the baby.
After the PGD is performed, the chances of achiving the desired sex is 99.9%.
The PGD process is the same as regular IVF however the embryos are kept in the laboratory for a 3-4 days longer to undergo biopsy. The following chromosomes are biopsied for abnormalities: X, Y, 13, 18 and 21.

North Cyprus Fertility Clinic is the first IVF clinic in Northern Cyprus to have two separate IVF laboratories and its own research center.  This allows us to provide  low cost gender (sex) selection IVF wiht state of the art technologies and high success rates. Our genetics laboratory  team includes a biogenetical engineer, PhD embryologist and lab assistant.

IVF and ICSI at North Cyprus IVF

In Vitro Fertilization is commonly referred to as IVF. IVF is the process of fertilization by manually combining an egg and sperm in a laboratory dish. When the IVF procedure is successful, the process is combined with a procedure known as embryo transfer, which is used to physically place the embryo in the uterus.

At North Cyprus IVF Center, In Vitro Fertilization requires a five step program which includes:

1- Monitoring and stimulating the development of healthy eggs in the ovaries: This is done through a process called ovulation induction where fertility medications are prescribed to control the timing of the egg ripening and to increase the chances of multiple egg growth. Since majority of our patients come from abroad, we prepare a well-charted protocol of treatment so that the medication can be taken without travelling to Cyprus. This way, the initial preparation process takes place in patient’s country of origin, shortening the time they are required to spend in Cyprus. We provide prescription for IVF medication for those who are unable to purchase them without a script. With advancements in technology, prescriptions we give can be used at websites like ivfmeds and patients can conveniently purchase their meds online. These medicines are easy to administer and we give you a very detailed chart on how to use them. This way, you are only expected to spend 7-8 days in Cyprus.

2- Retrieving the eggs: Once the eggs are developed through the first step, they are retrieved through a minor surgical procedure which uses ultrasound imaging to guide a hollow needle through the pelvic cavity. Sedation and local anesthesia are provided to remove any discomfort that you might experience. The eggs are removed from the ovaries using the hollow needle, which is called follicular aspiration.

3- Obtaining the sperm sample: This is usually done through ejaculation. Once the sperm is obtained, it is prepared for fertilization.

4- Fertilizing the eggs with the sperm: In a process called insemination, the sperm and eggs are placed in incubators located in the laboratory which enables fertilization to occur. In some cases where fertilization is suspected to be low, intracytoplasmic sperm injection (ICSI) may be used. The fertilized eggs are monitored for about three days until they become embryos grown enough to be transferred.

5- Embryo Transfer: The transfer process involves a speculum which is inserted into the vagina to expose the cervix. A predetermined number of embryos are suspended in fluid and gently placed through a catheter into the womb. This process is often guided by ultrasound. About 10-12 days after the transfer, a blood test and potentially an ultrasound will be used to determine if pregnancy has occurred.

At North Cyprus IVF, we provide low cost and affordable IVF treatment at a cost which is substantially below the cost of treatment in the U.S. and in Europe. Our reduced costs are a direct result of the volume of patients we serve and the high-tech equipment we use which substantially reduce human mistakes and allow us to be cost efficient. We are proud of our high success rates for all IVF, egg and sperm donation IVF and gender selection IVF treatments. For a quote, please send us a message using the contact form.

ICSI (Intracytoplasmic sperm injection) may be recommended when there is a reason to suspect that achieving fertilization may be difficult, even when the IVF procedures are used. ICSI is most often used with couples who are dealing with male infertility factors. Male infertility factors can include any of the following problems: low sperm counts, poor motility or movement of the sperm, poor sperm quality, or sperm that lack the ability to penetrate an egg.

At North Cyprus IVF, there are basically five simple steps to ICSI which include the following:

1. The mature egg is held with a specialized pipette.

2. A very delicate, sharp, and hollow needle is used to immobilize and pick up a single sperm.

3. This needle is then carefully inserted through the shell of the egg and into the cytoplasm of the egg.

4. The sperm is injected into the cytoplasm, and the needle is carefully removed.

5. The eggs are checked the following day for evidence of normal fertilization.

Once the steps of ICSI are complete and fertilization is successful, the embryo transfer procedure is used to physically place the embryo in the woman's uterus. Then it is a matter of watching for early pregnancy symptoms. The fertility specialist may use a blood test or ultrasound to determine if implantation and pregnancy has occurred.

Overall, ICSI is a procedure we follow in cases where we suspect the sperm count or motility or the sperm quality will not be sufficient to fertilize the eggs on its own, therefore, we assist the sperm in fertilizing the eggs. At Cyprus Fertility Clinic, we use ICSI as a standard procedure, hence, it does not cost anything extra on top of the low cost IVF program.

Talk about Success Rates!

Talk about Success Rates!

It is important to remember that every single IVF patient is individual and different.

Realistic success rates for treatment can only be provided after the individual couple has been evaluated closely.

Our success rates are based clinical pregnancies with a 3 embryo transfer.
Unfotunately since we have many patients from abroad, it is often hard to keep track of each one. It is common for International patients to lose touch once the treatment is complete and they have returned to their home country.

Of course we are always available to answer any of your questions pre and post treatment.

There are several reasons for failed attempts and here are a few of them:

embryo arrest
tubal disease
low sperm count/ sperm defects
reduced endometrial receptivity
embryonic defects
thin endometrium
immunological factors
genetic abnormalities
endometriosis
hydrosalpinge
thrombophilia
difficult embryo transfer
high estradiol levels
poor ovarian response

Quote from the San Diego Fertility Center :
Another problem with infertility treatment success statistics is the way in which they are reported. Should we report pregnancies based on each attempted cycle or overall success after 3 months or 6 months of infertility treatment? Should "success" be defined as a (+) pregnancy test or a live born child delivered 9 months later? Even the fertility experts can't agree and the organizations that collate success rates have changed their definitions over time. The most commonly reported success rate is a "clinical pregnancy"

Since this is a medical procedure unfortunately there is no way of knowing in advance what the reason for failure could be.

Short FAQ about Post IVF Treatment

Question :

I just wanted to double check that I will be taking 2 x Progynova twice a day and Crinone Gel twice a day, plus the Pregnacare and junior aspirin once a day until we are at the 12 week mark. Once we get past the first trimester do we just stop the Progynova (Estradiol Valerate)  and Crinone Gel or do a gradual wind down? And carry on with the Pregnacare and junior aspirin throughout my pregnancy?

Answer :

Please keep taking the same dose of medication until week 11 and for the final week, go down in dose of Progynova and the gel and stop altogether on week 12. You will keep using baby aspirin and Pregnacare until week 36, a few weeks before delivery.

Please be sure to have your prescription before leaving the clinic and email us if you have any further questions.

North Cyprus IVF Clinic - IVF-ICSI Questions

1- How do I know if IVF treatment is right for me?
This is something that would be decided with an IVF specialist. If you have been trying to get pregnant for at least a year without protection and have been unsuccessful, then it may be time to have some tests done. Infertility can be either caused by a male factor or a female factor or in some cases, caused by both.
For the male partner, it makes sense to have a semen analysis in order to check for sperm quality: sperm count, sperm motility and morphology. The semen analysis will indicate if there is any reason to suspect a male factor infertility.
For the female partner, the first round of testing would involve FSH, LH, Estradiol and Prolactine hormone tests. These tests are the indicators of the ovarian reserves and egg quality.
Even after the first round of testing, there are considerations to be made such as whether further testing would be needed, or whether IUI would be consulted prior to an IVF treatment.

2- Which tests do I need to have done prior to IVF treatment?
Your gynecologist will be able to make a better assessment based on your initial consultation. First, an ultrasound scan and a round of hormone testing will be called for. For the male partner, a semen analysis will be required. These are the standard tests. If your doctor believes that further infertility testing is necessary, then additional tests can be done.
Apart from infertility testing, IVF clinics usually ask for infectious disease testing as well. These are Anti-HIV, Anti-HCV, HbsAg, VDRL, Rubella IgG and sometimes CMV IgG. Some clinics require more tests than others, so it’s best to consult with the clinic which tests to have done prior to your IVF treatment.

3- How do I know if I can use my own eggs in IVF treatment?
Your hormone test results (especially FSH) will be indicative of the ovarian reserves. The hormone tests will allow your doctor to make an assessment of your ovarian reserves and estimate your likely response to a stimulation cycle.
Even though different clinics have different cut-offs for hormone levels, most clinics will not recommend using your own eggs with an FSH level higher than 15. In some cases, where the FSH test is inconclusive, the AMH test might be consulted.

4- What is the age limit for IVF treatment?
The age limit for IVF treatment varies from clinic to clinic and from country to country. Most clinics will not treat patients aged 45 and above while some will have an age limit of 50. Our age limit for IVF treatment is 45.

5- What does a high FSH result suggest?
Women are born with a set number of eggs. As they age the number and quality of eggs decrease. Basically FSH goes up in response to a negative feedback loop. Meaning as a woman's eggs start to diminish in quality or number, her body will in turn make more FSH to compensate for this loss. The egg itself communicates with the women's body to tell it to make certain hormones. So testing a woman's hormone levels can help determine the quantity and quality of eggs a woman has.. Anything higher than 14/15 will put the patient at a disadvantage in her response to stimulation medication. By measuring the amount of FSH, your doctor will be able to suggest specific treatments (such as in-vitro fertilization or intrauterine insemination) that could help you to conceive.

6- How do I interpret my hormone test results?
Usually, the specialist who asks you to get hormone testing will do the interpretation for you. However, what is important is to get the hormone tests done on day 3 of your menstrual period so that the results are most accurate. Most labs will give you a report with your hormone levels and reference values so that you will know whether your hormone levels are in the normal acceptable range or not.

7- Can I have IVF treatment with PCOS?
Absolutely! In fact, some women can get pregnant naturally even if they have PCOS. Most women change their diets and living styles as a permanent treatment method, and in most cases, these lifestyle changes can have a positive impact of fertility.
Women with PCOS have difficulty ovulating properly, so you may have to use medication to stimulate ovulation and to regulate your menstrual periods. Your doctor may also prescribe injections such as an HCG shot to further stimulate your ovaries. However, if these changes do not allow you to fall pregnant naturally, or if you have not responded well to conventional medication, the IVF might be called for.

8- What is the IVF success rate for women with PCOS?
As long as the patient with PCOS is subjected to a successful stimulation cycle, and as long as some good quality eggs are retrieved, women with PCOS will have no problem falling pregnant. In fact, the rate of success will not be that much different from women without PCOS.

9- How does endometriosis affect IVF treatment?
The most important handicap of women with endometriosis is reduced egg quality. As a consequence, the rate of fertilization is much lower compared to women with no fertility issues. Therefore, the main problem is creating of the embryo rather than implantation. If you have gone through a few IVF cycles without success due to fertilization failures, the most sensible option might be to opt for egg donor IVF.

10- What is the IVF success rate for women with endometriosis?
Endometriosis is a condition in which the tissue endometrium, normally lining the uterus, grows on different organs outside the uterus.
In severe cases, formation of connecting tissues around endometriosis near the Fallopian tubes or ovaries may reduce fertility. Endometriosis can affect egg quality substantially, therefore, reducing the likelihood of a woman of becoming pregnant with her own eggs. In some cases, IVF using an egg donor is the only solution for women with chronic endometriosis.

11- Does IVF medication have any side effects?
There is no consensus on the possible side-effects of IVF medication. For quite a long time, IVF medication were thought of possibly causing ovarian cancer, but recent studies provide evidence that there is no direct link between IVF medication and cancer.
If side effects are experienced, they are usually very mild and short-lasting. Some patients experience feeling emotional and/or bloated. These are directly related to the changing hormone levels within the body.

12- How many eggs are usually retrieved in an IVF cycle?
This depends on your hormone levels, ovarian reserves, and how well you react to stimulation medication. An optimal stimulation cycle should result in an average of 10-12 good quality eggs to be fertilized. This allows the patient to have an optimal number of embryos (depending on sperm quality) to transfer and perhaps to freeze for future cycles.

13- How many embryos are put back in an IVF treatment?
This would depend on the IVF laws of a country. Some countries only allow for a single embryo transfer, while some countries have different numbers for different age groups. At younger ages, you are more likely to succeed with a lower number of embryos transferred; however, as you get older, it might be necessary to transfer multiple embryos in order to achieve pregnancy.
While single embryo transfers reduce the chances of IVF success, multiple embryo transfers increase the likelihood of a multiple pregnancy, therefore, you should plan carefully when you get to that stage in your IVF treatment. At North Cyprus IVF, our embryo transfer limit is 3 embryos.

14- What happens to unused embryos after the treatment?
Most clinics will offer you the option to freeze any unused embryos for future use if you have a sufficient amount of unused embryos of good quality. You will be able to have your unused embryos frozen for future cycles at a small fee. This will give you the opportunity to have future IVF attempts with minimal preparation and costs.
If the clinic does not offer cryopreservation (freezing), or if you do not want to have your embryos frozen, they are usually disposed of, or are used in clinical trials. The IVF specialist would make this clear prior to treatment.
At North Cyprus IVF, we offer our patients the option of freezing any unused embryos they have for future use.

15- Semen analysis shows zero sperm count, can we try IVF?
Even though a semen analysis may indicate a zero sperm count, it might be the case where there are live sperm cells in the testes. This can only be identified through minor surgical procedures like TESE/TESA/Micro-TESE and the like. These procedures are used to surgically remove a piece of tissue containing sperm cells. If there are live sperm cells, they will be used in IVF treatment along with ICSI to assist the sperm with fertilization.
In cases where these surgical procedures yield no results, IVF using a sperm donor becomes the only viable option.

16- I have reduced ovarian reserves, what type of treatment is right for me?
There is no single answer to this question. There are different levels of reduced reserves. You can have sub-optimal reserves, meaning you could be producing sufficient amount and quality of eggs with a good stimulation cycle, or you could be pre-menopausal, suggesting a very low chance of success.
An IVF specialist will typically ask for your hormone test results and make an assessment accordingly. Based on your hormone levels, (s)he will be able to prescribe you the right mode of treatment and the right dose of medication for the most efficient stimulation.

Gender Selection (PGD/MicroSort) Questions

Why do I need to get hormone tests done if I already have children?
The hormone tests are requested in order for the Dr. to better understand the quality and quantity of eggs and provide a personalized and detailed treatment for each individual patient.

Why do I need to have an ultrasound before I begin treatment?
The ultrasound is requested in order to make sure there are no ovarian cysts, polyps or anything else that might stand in the way of a successful IVF cycle.

What is the difference between Microsort and PGD?
Microsort is a pre-fertilization gender selection method which is used to sort sperm into male and female producing cells. It is about 80% accurate for selection of a boy and 90% accurate for selection of a girl. This way, we can make sure majority of your embryos to be fertilized will be of desired sex.
PGD, on the other hand, is a post-fertilization gender selection method where the embryos that are fertilized are subjected to a genetic analysis for sex selection. Even though PGD provides 99.9% accuracy for gender selection, and it also tests for majority of possible genetic disorders, making sure your child does not have them, it cannot guarantee that majority of your embryos will turn out to be of desired sex.
The best gender selection method is to combine microsort with PGD so that you can be sure that you will have a number of embryos which are of desired sex, anda re free of most genetic conditions.

What is the HCG trigger injection for?
This shot is for women who are using their own eggs and it helps release the mature eggs from the follicles and is usually done 35 hours before egg collection. It is the final step in assisting the maturation process and ensure the eggs are released from the follicles.
This is generally done in Cyprus but can be done before traveling in order to shortent the stay.

What does Gonal F do?
This is a follicle stimulating hormone (FSH) to help increase the number of growing follicles and their development. The dosage will be prescribed based on the inital hormone test results and varies from individual to individual. This is in fact one of the reasons this test is prescribed prior to starting treatment even if the patient has no infertility issues and has conceived children successfully before.

What does Menopur do?
Menopur is also used for follicle growth and development and contains follicle stimulating hormone as well as luteinising hormone (LH). This is a combination we prefer for women older than 35 years of age as quite a large number of studies, and our experience, suggest that LH can be a good supplement for optimizing egg quality for women over 35 years of age.

Can I travel with the injections?
The injections will need to be checked in with your luggage but there should be no problems traveling with them.

Do we have to use the accommodation offered in your packages?
Some patients decide they would prefer to select their own accommodation which is no problem for us. If you would like to still take advantage of the free ground transfers however, it is preferable that your hotel be in Nicosia (Lefkosa) North Cyprus.

Why do I need to have scans during medication use?
The scans for Egg Donation patients are requested in order to make sure that the lining of the womb is ready for the embryo transfer.
The scans for women using their own eggs (IVF with ICSI and Gender Selection/PGD) is to ensure the follicles are developing well before egg collection.
It is important to let us know the results of the scans so that we can adjust your treatment schedule and medication intake if necessary.

How are the injections done?
The Gonal F injections should be done subcutaneously either in the abdomen/stomach area, naval area or upper thigh where there is excess skin and fatty tissues. It is recommended to vary the injection site.
Please watch the videos below for preparing and administering the medication.
http://www.youtube.com/watch?v=OMklbInsQGk
http://www.youtube.com/watch?v=AHEyyB0IKNY

When are the eggs collected?
The eggs are generally collected on day 15 of the cycle after an ultrasound and the trigger injection.

When is the embryo transferred?
We usually prefer to transfer embryos on day 18 of menstrual period, which is the best implantation window. However, days 17-20 are acceptable embryo transfer dates. If patients are undergoing PGD analysis, then the transfer is delayed by 2 days until the embryos reach the blastocyst stage and are transferred on day 20. This of course might vary from patient to patient. We can transfer up to 3 embryos at our clinic.

What chromosomes are examined with PGD?
During the PGD biopsy we test chromosomes X, Y, 13, 18 and 21 which are causes of close to 90% of abnormalities.

What happens if we have left over embryos?
Many patients will choose to freeze the embryos in case they need it in the future, other patients will ask for the remaining embryos to be destroyed or used during our clinical studies so we can keep offering you the highest quality service through our own research and clinical studies.

When do I do the pregnancy test?
The pregnancy blood test is suggested 12 days after the embryo transfer.

What happens after a positive pregnancy result?
After a positive result you will continue to take medication for 11 weeks after the embryo transfer. Normally this will be Crinone Gel (Progesterone) and Aspirin. Prenatal vitamin supplements can be used for the entire pregnancy term.
