IVF Treatment with Cytoplasmic Transfer


Cytoplasmic transfer is not a method of treatment on its own. Indeed it is a procedure used with IVF/ICSI Treatment. As one of the latest innovations in IVF Treatment, it was created mainly to help women who experience infertility because of deficient or damaged mitochondria which is contained within an egg's cytoplasm. Mitochondria provides the necessary energy for cell growth as well as efficient cell division. Therefore a deficiency in mitochondria will prevent successful cell growth and cell division. Consequently, it may cause recurrent implantation failures, high levels of embryo fragmentation, poor embryo quality and development, and last but not least a failed IVF.

The main difference of IVF/ICSI involving a cytoplasmic transfer and a regular IVF/ICSI is that, the former uses not only the patient's own eggs but also donor eggs in order to obtain healthy mitochondria from young and healthy donor eggs. Healthy mitochondria will aid rejuvenate the patient's eggs and thus, maximize the chances of success.

As North Cyprus IVF, we have been offering Cytoplasmic Transfer to our patients since 2012. Even though we do not have sufficient numbers to publish significant statistics, it would be fair to mention that cytoplasmic transfer potentially adds a few percentages to the chances of success for women in their early 40s. Still, please note that; cytoplasmic transfer cannot correct any genetic defects related with the egg that may have caused by older age. We recommend this procedure to patients in their mid 40's. However, given the fact that possibilities of success with own eggs declines considerably after mid 40s, we also recommend Donor Egg IVF treatment especially after a certain age.

Defining Infertility






We can simply define Infertility as "inability to conceive" or "inability to carry pregnancy to a full-term". Close to ten percent of the western population, those between the ages of 15-44, having infertility problems. Although many assume infertility is female problem, female and male infertility have almost the same level of prevalence within the society.

While primary infertility definition includes couples who have been trying to get pregnant for the last year but incapable to do so, secondary infertility involves couples who have previously conceived but are not able to do so any more. Some factors that may cause secondary infertility are ; age, premature menopause, exposure to radiation therapy, chemotherapy, surgical complications, bad diet and more.

Initial fertility testing differ for each couple depending on the factors such as; time they have been trying to conceive, their age, personal history of fertility treatments, medication protocols used in these treatments, their history with miscarriages and many more.

Please, keep in mind that there is no universal infertility treatment that can be applied each and every patient. Treatments are customized and the one you need cannot be identified until you have been thoroughly checked and tested.

MALE INFERTILITY TESTING




In male infertility assessments, a semen analysis, ideally on the third or for day of abstinence (no sexual activity), is the first step. The capacity of the sperm to fertilize the egg is measured through a sperm sample which reveals parameters like; sperm count, concentration, motility, morphology, round cell count, pH and more. Without a doubt, some other tests might be required as well depending on patient's personal history of infertility, testing and or treatment


The WHO criteria for normal sperm values is listed as follows;

Volume: > 2.0 ml
Concentration: >20 million/ml
Motility: > 50%
Morphology: >30% with normal morphology
White Blood Cells (Round Cells): < 1 million/ml
pH: 7.2-7.8

Required Tests - IVF


Certain tests are required before we can proceed with IVF treatment. Some of these tests are common to every IVF patient regardless of the treatment option they choose. These are the infectious contagious disease tests including:

HbsAg, Anti-HCV and Anti-HIV tests both for the male and the female partner.

We also require further testing forVDRL and Rubella IgG of the female partner.

The aim of infectious disease testing is to make sure that these diseases are not passed onto the newborn (or the surrogate in the case of gestational surrogacy). These tests can be done prior to your arrival at our clinic in Cyprus or once you arrive at our clinic. It is, however, required that we see these test results before the embryo transfer stage.

We also require certain tests for patients undergoing conventional IVF, Sperm Donation IVF or Gender Selection IVF. These are the hormone tests to be done on the 3rd day of the menstrual cycle of the female patient. The aim of the hormone tests is to identify egg quality and quantity produced by the female patient. These test results will tell us about the usability of the female patient's eggs in the IVF procedure. You should keep in mind that age is also a very important factor. A female above the age of 40 with acceptable test results may still have a very low chance of IVF due to older age. The hormone tests we require include:

FSH: Follicle stimulating hormone (FSH) is one of the most important hormones involved in the natural menstrual cycle as well as in pharmacological (drug-induced) stimulation of the ovaries. It is the main hormone involved in producing mature eggs in the ovaries. We consider normal FSH level to be anything less than 9. As levels go above 9 we often see a reduction in response to ovarian stimulating drugs

LH: The Luitenizing Hormone (LH) test is used for a variety of reasons. It is typically used to detect when a woman is ovulating, so that eggs can be retrieved for an IVF procedure. On Day 3, normal LH values are between 3 and 10 mIU/mL. LH levels can also be artificially raised by ovulation inducing drugs such as clomiphene citrate ( clomid).


E2: Performed on Day 3 of your menstrual cycle, the estradiol test is one of the most common fertility testing procedures. Most women undergoing fertility treatments will receive the estradiol test. Women have particular levels of estradiol during different stages of their menstrual cycle. By analyzing these levels on Day 3, your reproductive endocrinologist can determine your ovarian reserve. Normal Day 3 estradiol levels tend to be around 80 pg/mL or lower. If your estradiol levels are higher than this, than this could indicate a problem with your ovulation

PRL: Prolactin is the “milk producing hormone” secreted by the pituitary gland. Elevated prolactin levels must be reduced for successful conception to occur.

Patients undergoing conventional IVF, Gender Selection IVF or Egg Donation IVF are also asked to provide a semen analysis report including sperm count, motility and morphology. This is to make sure that the male partner is able to produce healthy sperm and can be used in the IVF procedure.

Microsort IUI



IUI (intrauterine insemination) treatment is generally recommended for patients in their 20’s who prefer a non-invasive alternative to IVF.

A mild dose of medication is prescribed for ovulation in order to increase the number of eggs that are produced. The male sperm is then sorted into X and Y with the Microsort procedure and inserted into the uterus once the eggs have matured and are ready for fertiization.

This method is 90% accurate for female and 75% accurate for male offspring.
As the eggs are not retrieved from the female, there is no biopsy or genetic testing performed to ensure 100% accuracy.

The success rates for IUI are about 5-30% depending on the age of the female and multiple tries might be required until pregnancy is achieved.

I am so happy to let you know that we had our pregnancy blood test result yesterday and it's a positive!
We are over the moon, yet still cautious and hope the next 10 weeks will go smoothly.

I just wanted to double check that I will be taking 2 x Progynova twice a day and Crinone gel twice a day, plus the Pregnacare and junior aspirin once a day until we are the 12 week mark. Once we get past the first trimester do we just stop the Progynova and Crinone or do a gradual wind down? And carry on with the Pregnacare and junior aspirin throughout my pregnancy?

British Channel 4 Documentary on Multiples and IVF Abroad

Dragonfly is a factual BAFTA-award winning independent production company, our programmes include Extreme A&E, One Born Every Minute and The Family.

Channel 4 have commissioned us to make a new one-off documentary about multiple births. The aim is to show viewers what it is like to have a multiple birth, by focusing on the human and medical stories of parents. We also hope to give an understanding of the complexities of multiple births and highlight the specialist work of the healthcare professionals who work with multiples cases.  In practice, this means filming the journeys of women, babies and families at a number of maternity units in the UK.

We are very keen to speak to British mums expecting multiples who have had IVF abroad and ended up falling pregnant with more than one baby!

We heard about the clinic after a failed ivf treatment at a well known English clinic, which was lovely with high tec everything, but also a high tec price!! A friend told us about North Cyprus IVF clinic as she had TWO friends pregnant from their treatment there, one with twins and one with triplets! The clinic were professional and friendly. The decor of the clinic is a bit chintzy which is not what we expected but the places that matter like the operating theatre were stare of art. The staff were friendly and although not many speak Enflish those that did were fluent and helpful. Overall I was very very impressed and would recommend them As I have been, to anyone who needs treatment. Gender (Sex) Selection in the UK

Gender selection in the United Kingdom is prohibited by law, therefore, UK patients are not able to choose the gender of their babies for family balancing purposes within the UK. However, UK patients are able to travel abroad for this treatment option.

Given the difficulty of traveling for great lenghts of time for the working people, we have established links with IVF specialists, and scan clinics in the UK, so that our patients can have their preliminary work performed in the UK and then travel to Cyprus for only about a week. This way, our patients feel more comfortable receiving pre-treatment with a local doctor under our supervision and only travel for a short period of time to finalize their treatments.

Having this option, UK patients are able to take advantage of IVF technologies in Cyprus without compromising the legal issues associated with gender selection treatment.

Gender Selection in North Cyprus


New technologies are now allowing some couples to choose the sex of their child before pregnancy. Many couples wish to have a child of a particular sex in order to prevent certain medical illness or to help balance their families.

PGD is one of the most successful methods when it comes to gender selection. This is a complex procedure that involves the creation of embryos that are then analyzed for cell structure and DNA. Embryos are created in a laboratory and allowed to divide. After three or four days, one cell from each dividing embryo is removed and analyzed for DNA and genetic material. Once the sex of the embryos are determined, only those embryos of the desired sex are implanted into the mother’s uterus through IVF. PGD is highly successful, giving you a 99% chance of having a child of the desired gender.

Gender Selection IVF works very much like the IVF procedures explained in the “IVF” section under “Our Services”. However, the embryos that are formed using the IVF procedures undergo genetic testing known as PGD before they are transferred into the uterus. Therefore, in gender selection IVF, the five step IVF procedure requires a sixth step, which comes right before the embryo transfer step.

North Cyprus Fertility Clinic offers low cost, high success Gender (sex) selection IVF by using state of the art technology in fertility treatment and a team of experts. Our genetics laboratory employs a biogenetical engineer, two PhD embryologists and a lab assistant. We are the very first IVF clinic with two separate IVF laboratories and its own research center.

North Cyprus IVF is now the second location in the world to offer Microsort for gender selection.
Microsort is a technology that sorts sperm into X and Y chromosomes prior to fertilizing the eggs, thus, allowing you to have more male or female embryos after fertilization. Microsort coupled with PGD for gender selection will offer dramatically high chances of success compared to PGD alone.
We have started taking applications for our Microsort program. We will be offering microsort starting in July. However, due to the large volume of demand for this procedure, we will be having a waiting list. The sooner you apply, the sooner you have a chance of being admitted into our microsort program. The following are the three types of alternative gender selection methods we are able to offer through microsort.

IUI + Microsort for Gender Selection:
This is the least invasive gender selection method. Female partner is prescribed a mild dose of medication for ovulation induction so that multiple eggs are produced in the ovaries. Male partner's sperm is sorted via the microsort technology into those that produce boy and girl offsprings. The sorted sperm is injected into the uterus when the eggs are matured and ready for fertilization. This gender selection method is about 90% accurate for female offsprings and 75% accurate for male offsprings. Since eggs are not retrieved from the ovaries, there is no way of biopsying and genetically testing the embryos for sex with 100% accuracy. IUI is usually recommended for women in their 20s as the patient may be required to undergo a series of IUI procedures until pregnancy can be achieved. Success rates with IUI vary between 5-30% depending on the age of the female partner.

IVF + Microsort for Gender Selection:
This treatment method is very much like conventional IVF apart from the fact that male partner's sperm is sorted into those that produce male and female offsprings through the microsort procedure. The microsort sperm is used to fertilize the eggs that are retrieved from the female partner's ovaries at the end of the ovulation induction process. The eggs are fertilized in a laboratory environment and are placed in an incubator until they reach a certain level of development and are then transferred into the uterus for implantation. The medication used in IVF treatment is slightly different from the IUI protocol. Patients who are in their late 20s or in their 30s are often advised to go straight for IVF treatment rather than IUI for higher chances of success.

IVF + Microsort + PGD for Gender Selection:
This is the most effective and most accurate gender selection method. All the steps involved in this treatment are exactly the same as in IVF + Microsort. The only difference is, once the embryos reach a certain level of development (usually 8-cell stage), they are biopsied for genetic screening and gender selection.
