Female Infertility - Causes and Investigation


 Most common causes of infertility in women are; absence of ovulation or insufficient quality of the eggs (oocytes) for fertilization. Additionally, many factors such as; age, stress, excessive use of tobacco and alcohol, sexually transmitted and infectious diseases, being overweight or underweight, athletic training, lack of the essential nutrients resulting from a poor diet and any factors leading to hormonal imbalances may act on a woman's fertility.

Aging has a crucial effect on a woman's fertility. After the age of 30, a woman's chance of having a baby decrease expeditiously each year. Mainly because the health of a woman's ovaries to release eggs and health of the eggs declines whereas, risks of having a miscarriage, as well as risks arising from a woman's personal health problems which may interfere with fertility increases. Therefore women in their 30's who have been trying to get pregnant for six months or longer should immediately consult their doctors for a complete fertility evaluation. As for most healthy women under the age of 30, should not worry unless they have been trying for at least a year.

Infertility testing and evaluation starts at your gynecologist's, where a sonogram will be used to find out the number of potential eggs in each ovary on the 2nd or 3rd day of your menstrual period. Besides, hormone testing will be useful in order to make an evaluation of your ovarian function including FSH, LH, Estradiol, Prolactin, TSH and AMH. Depending on the results, the most appropriate course of action will be decided.

La cryoconservation des ovocytes
Les progrès récents dans le dans le processus de fécondation in vitro ont permis d'assurer le succès congeler , stocker et décongeler plus tard et féconder des ovules humains cryoconservés . Cryoconservation des ovocytes s'adresse à trois groupes particuliers de femmes : les personnes atteintes d' un cancer qui n'ont pas encore commencé la chimiothérapie ou la radiothérapie , et ceux en cours de traitement avec les technologies de reproduction assistée qui ne considèrent pas la congélation des embryons une option , et ceux qui voudraient préserver leur capacité future d'avoir des enfants , soit parce qu'ils ne possèdent pas encore un partenaire , ne sont pas prêts à avoir un bébé encore, mais voudrait garder cette option pour l'avenir , ou pour d'autres raisons personnelles ou médicales . Le procédé de récupération d'oeuf pour la cryoconservation d'ovocytes est la même que celle pour la fécondation in vitro . Cela inclut plusieurs semaines d' injections d'hormones et à la contraception hormonale afin de stopper l'ovulation , suivie par plus d'injections d'hormones pour stimuler les ovaires et mûrir plusieurs ovules . Quand les oeufs ont mûri , hormone supplémentaire est donné et les oeufs sont enlevés avec une aiguille guidée par échographie par voie vaginale . La procédure est réalisée sous sédation . Les œufs sont immédiatement congelés . Ovules congelés peuvent être conservés aussi longtemps que le patient souhaite , mais cela doit être indiqué avec précision , sinon , les oeufs congelés qui ne sont pas utilisées dans un délai de deux ans sont déchargées.


La cryoconservation d'embryons
La cryoconservation d'embryons ( congélation des embryons ) est la congélation des embryons «excédentaires» pour une utilisation dans des cycles de FIV ultérieures. Cette option est souvent explorée par les couples en traitement de FIV où les embryons surnuméraires qui ne seront pas transférés dans l'utérus sont congelés pour une utilisation future si la tentative de FIV échoue. Même lorsque la tentative de FIV ne manque pas et le couple est capable de concevoir , les embryons congelés peuvent être utilisés pour la seconde grossesse. Le principal avantage de l'utilisation d'embryons cryoconservés dans les futurs cycles de FIV est que la femelle n'a pas à subir une induction de l'ovulation , ce qui réduit aussi considérablement les coûts des médicaments. Les taux de réussite sont variables en fonction des caractéristiques des patients et la qualité des embryons .
Cela pourrait aussi être une option pour les couples qui ne souhaitent pas tomber enceinte tout de suite mais je voudrais préparer leurs embryons à l'avance pour une utilisation future . De cette façon , ils peuvent éviter des taux de réussite inférieurs de conception à un âge avancé .

Cryoconservation du sperme
La congélation du sperme ( cryopréservation ) est une méthode utilisée pour les patients masculins qui suivent un traitement de chimiothérapie pour un cancer ou de maladies similaires . Cela permet au patient de conserver leur sperme pour une utilisation future et éviter les risques d'infertilité associés à la chimiothérapie .

Egg Donation Program


Egg Donation IVF is the treatment method for women who do not have healthy eggs for conception. The following list indicates women who are ideal candidates for egg donor IVF treatment at North Cyprus IVF clinic:

- Women who are going through premature menopause
- Women who have had their ovaries removed and therefore do not produce eggs
- Women who have undergone cancer treatment and have damaged ovaries
- Women who were born without functioning ovaries (Turner's syndrome)
- Women with poor ovartian reserves
- Women with hereditary genetic disorders
- Women who have had multiple miscarriages due to chromosomal abnormalities

Who Are Egg Donors at North Cyprus IVF?

Our egg donor base is consisted predominantly of international students studying at various universities in Cyprus. As a result, majority of our egg donors are between 20-24 years of age. This is precisely why our success rates are considerably higher than most of the IVF clinics in the world. Our egg donors are very young and most of them have previous positive donation histories.

In general, our egg donor selection criteria can be summarized as follows:
- women between 20-29 years of age.
- women with BMI's between 19-24
- women with no known hereditary genetic diseases in family.

- women who do not use drugs and do not smoke

Required Tests - IVF


Certain tests are required before we can proceed with IVF treatment. Some of these tests are common to every IVF patient regardless of the treatment option they choose. These are the infectious contagious disease tests including:

HbsAg, Anti-HCV and Anti-HIV tests both for the male and the female partner.

We also require further testing forVDRL and Rubella IgG of the female partner.

The aim of infectious disease testing is to make sure that these diseases are not passed onto the newborn (or the surrogate in the case of gestational surrogacy). These tests can be done prior to your arrival at our clinic in Cyprus or once you arrive at our clinic. It is, however, required that we see these test results before the embryo transfer stage.

We also require certain tests for patients undergoing conventional IVF, Sperm Donation IVF or Gender Selection IVF. These are the hormone tests to be done on the 3rd day of the menstrual cycle of the female patient. The aim of the hormone tests is to identify egg quality and quantity produced by the female patient. These test results will tell us about the usability of the female patient's eggs in the IVF procedure. You should keep in mind that age is also a very important factor. A female above the age of 40 with acceptable test results may still have a very low chance of IVF due to older age. The hormone tests we require include:

FSH: Follicle stimulating hormone (FSH) is one of the most important hormones involved in the natural menstrual cycle as well as in pharmacological (drug-induced) stimulation of the ovaries. It is the main hormone involved in producing mature eggs in the ovaries. We consider normal FSH level to be anything less than 9. As levels go above 9 we often see a reduction in response to ovarian stimulating drugs

LH: The Luitenizing Hormone (LH) test is used for a variety of reasons. It is typically used to detect when a woman is ovulating, so that eggs can be retrieved for an IVF procedure. On Day 3, normal LH values are between 3 and 10 mIU/mL. LH levels can also be artificially raised by ovulation inducing drugs such as clomiphene citrate ( clomid).


E2: Performed on Day 3 of your menstrual cycle, the estradiol test is one of the most common fertility testing procedures. Most women undergoing fertility treatments will receive the estradiol test. Women have particular levels of estradiol during different stages of their menstrual cycle. By analyzing these levels on Day 3, your reproductive endocrinologist can determine your ovarian reserve. Normal Day 3 estradiol levels tend to be around 80 pg/mL or lower. If your estradiol levels are higher than this, than this could indicate a problem with your ovulation

PRL: Prolactin is the “milk producing hormone” secreted by the pituitary gland. Elevated prolactin levels must be reduced for successful conception to occur.

Patients undergoing conventional IVF, Gender Selection IVF or Egg Donation IVF are also asked to provide a semen analysis report including sperm count, motility and morphology. This is to make sure that the male partner is able to produce healthy sperm and can be used in the IVF procedure.

Talk about Success Rates!

It is important to remember that every single IVF patient is individual and different.

Realistic success rates for treatment can only be provided after the individual couple has been evaluated closely.

Our success rates are based clinical pregnancies with a 3 embryo transfer.
Unfotunately since we have many patients from abroad, it is often hard to keep track of each one. It is common for International patients to lose touch once the treatment is complete and they have returned to their home country. 

Of course we are always available to answer any of your questions pre and post treatment.

There are several reasons for failed attempts and here are a few of them:

embryo arrest
tubal disease
low sperm count/ sperm defects
reduced endometrial receptivity
embryonic defects
thin endometrium
immunological factors
genetic abnormalities
endometriosis 
hydrosalpinge
thrombophilia
difficult embryo transfer
high estradiol levels
poor ovarian response

Another problem with infertility treatment success statistics is the way in which they are reported. Should we report pregnancies based on each attempted cycle or overall success after 3 months or 6 months of infertility treatment? Should "success" be defined as a (+) pregnancy test or a live born child delivered 9 months later? Even the fertility experts can't agree and the organizations that collate success rates have changed their definitions over time. The most commonly reported success rate is a "clinical pregnancy"

Since this is a medical procedure unfortunately there is no way of knowing in advance what the reason for failure could be.
