Gender Selection (PGD/MicroSort) Questions

Why do I need to get hormone tests done if I already have children?
The hormone tests are requested in order for the Dr. to better understand the quality and quantity of eggs and provide a personalized and detailed treatment for each individual patient.

Why do I need to have an ultrasound before I begin treatment?
The ultrasound is requested in order to make sure there are no ovarian cysts, polyps or anything else that might stand in the way of a successful IVF cycle.

What is the difference between Microsort and PGD?
Microsort is a pre-fertilization gender selection method which is used to sort sperm into male and female producing cells. It is about 80% accurate for selection of a boy and 90% accurate for selection of a girl. This way, we can make sure majority of your embryos to be fertilized will be of desired sex.
PGD, on the other hand, is a post-fertilization gender selection method where the embryos that are fertilized are subjected to a genetic analysis for sex selection. Even though PGD provides 99.9% accuracy for gender selection, and it also tests for majority of possible genetic disorders, making sure your child does not have them, it cannot guarantee that majority of your embryos will turn out to be of desired sex.
The best gender selection method is to combine microsort with PGD so that you can be sure that you will have a number of embryos which are of desired sex, anda re free of most genetic conditions.

What is the HCG trigger injection for?
This shot is for women who are using their own eggs and it helps release the mature eggs from the follicles and is usually done 35 hours before egg collection. It is the final step in assisting the maturation process and ensure the eggs are released from the follicles.
This is generally done in Cyprus but can be done before traveling in order to shortent the stay.

What does Gonal F do?
This is a follicle stimulating hormone (FSH) to help increase the number of growing follicles and their development. The dosage will be prescribed based on the inital hormone test results and varies from individual to individual. This is in fact one of the reasons this test is prescribed prior to starting treatment even if the patient has no infertility issues and has conceived children successfully before.

What does Menopur do?
Menopur is also used for follicle growth and development and contains follicle stimulating hormone as well as luteinising hormone (LH). This is a combination we prefer for women older than 35 years of age as quite a large number of studies, and our experience, suggest that LH can be a good supplement for optimizing egg quality for women over 35 years of age.

Can I travel with the injections?
The injections will need to be checked in with your luggage but there should be no problems traveling with them.

Do we have to use the accommodation offered in your packages?
Some patients decide they would prefer to select their own accommodation which is no problem for us. If you would like to still take advantage of the free ground transfers however, it is preferable that your hotel be in Nicosia (Lefkosa) North Cyprus.

Why do I need to have scans during medication use?
The scans for Egg Donation patients are requested in order to make sure that the lining of the womb is ready for the embryo transfer.
The scans for women using their own eggs (IVF with ICSI and Gender Selection/PGD) is to ensure the follicles are developing well before egg collection.
It is important to let us know the results of the scans so that we can adjust your treatment schedule and medication intake if necessary.

How are the injections done?
The Gonal F injections should be done subcutaneously either in the abdomen/stomach area, naval area or upper thigh where there is excess skin and fatty tissues. It is recommended to vary the injection site.

When are the eggs collected?
The eggs are generally collected on day 15 of the cycle after an ultrasound and the trigger injection.

When is the embryo transferred?
We usually prefer to transfer embryos on day 18 of menstrual period, which is the best implantation window. However, days 17-20 are acceptable embryo transfer dates. If patients are undergoing PGD analysis, then the transfer is delayed by 2 days until the embryos reach the blastocyst stage and are transferred on day 20. This of course might vary from patient to patient. We can transfer up to 3 embryos at our clinic.

What chromosomes are examined with PGD?
During the PGD biopsy we test chromosomes X, Y, 13, 18 and 21 which are causes of close to 90% of abnormalities.

What happens if we have left over embryos?
Many patients will choose to freeze the embryos in case they need it in the future, other patients will ask for the remaining embryos to be destroyed or used during our clinical studies so we can keep offering you the highest quality service through our own research and clinical studies.

When do I do the pregnancy test?
The pregnancy blood test is suggested 12 days after the embryo transfer.

What happens after a positive pregnancy result?
After a positive result you will continue to take medication for 11 weeks after the embryo transfer. Normally this will be Crinone Gel (Progesterone) and Aspirin. Prenatal vitamin supplements can be used for the entire pregnancy term.

Nous avons entendu parler de la clinique après un traitement de FIV a échoué dans une clinique anglaise bien connue, ce qui était très agréable avec tout haut tec, mais aussi un prix élevé tec! Un ami nous a parlé de Chypre du Nord clinique de la FIV comme elle avait deux amies enceintes de leur traitement il, une avec lits jumeaux et une avec des triplés! Nous avons décidé après mûre réflexion pour essayer de la clinique et je suis tellement heureux que nous avons fait comme nos jumeaux sont dus en April.We essayé 5 ans pour commencer notre famille. La clinique semblait un peu daté après notre traitement au Royaume-Uni, mais le personnel était toujours serviable et nos résultats en ce qui me concerne parlent d'eux-mêmes. La clinique était professionnel et sympathique. Le décor de la clinique est un peu rustique qui n'est pas ce que nous attendions, mais les lieux qui comptent comme le bloc opératoire étaient regard de l'art. Le personnel était sympa et bien que pas beaucoup parlent Enflish ceux qui ne parlaient couramment et utile. Globalement, j'ai été très impressionné et je le recommande comme je l'ai été, pour ceux qui ont besoin de traitement. Nous avions FIV en utilisant nos propres ovules et les spermatozoïdes et atteint notre taux de fécondité le plus élevé que nous ayons jamais eu et mes triplés maintenant âgé de 10 mois.

Tandem IVF Cylce at North Cyprus IVF








Gaining great popularity within the last few years, Tandem Cycling allows women with lowered ovarian reserves to have a shot at pregnancy using their own eggs, but at the same time having a back-up egg reserves through the use of an egg donor. A tandem cycle allows you to receive a lower dose of medication since you will only be needing a few good quality eggs. At the end of this process, you get to decide what combination of your own eggs and donor eggs to use. This also allows you to have a go with your own eggs first and freeze donor's eggs for a future cycle without facing the cost of two separate cycles.

A tandem cycle is a great option for older women and for women with elevated FSH levels. A tandem cycle can be seen as investing in two different stocks to divide the risk of your investment, so the risk is minimized. A tandem cycle gives you the option of using your own eggs in your treatment, bit if worst comes to worst, you have your back-up eggs that comes from your egg donor, so you do not have to go through yet another IVF cycle.

At North Cyprus IVF, we have been providing tandem cycle treatment for some time with some great success stories. Especially when a combination of your eggs and your donor's eggs are used, the chances of success tend to go up dramatically. Furthermore, the fact you are given a chance that one of your own embryos with your own genetic material could be the one embryo to produce pregnancy, you might feel more positive about using a tandem cycle as opposed to using solely donor eggs.

The main difference between egg donation and a tandem cycle is that, the patient also receives medication to stimulate her ovaries in order to produce eggs. At the end of the process, both sets of eggs are collected and based on the outcome, the patients are offered a variety of alternatives.

However, just like in an egg donation cycle, the patients are asked to provide with the following information for an accurate donor matching process:

- Female patient's physical features (skin, hair and eye color)

PGD - Gender (Sex) Selection IVF in Cyprus



At North Cyprus IVF, one of the treatment options that we offer is PGD for genetic testing and family balancing purposes.

Gender (sex) selection IVF is also a growing concept in the field of IVF. Allowing parents to choose the sex of their baby prior to conception has raised a number of debates on the issue, but regardless of what has been said and written on the subject, sex selection has been a way balancing families and eliminating genetic disorders that are associated with X or Y chromosomes.

PGD genetic testing is also an option when families would like to select the gender of their baby for family balancing purposes. The embryo biopsy will not only allow us to select embryos that are free of genetic abnormalities, but will also allow us to determine the sex of the embryos. Once PGD is performed, the chance of having a baby of desired sex is very close to 100% once pregnancy is achieved. This process is also known as gender selection (sex selection) IVF.

PGD works very much like conventional IVF. The only difference is; embryos that are formed in the laboratory are subjected to a biopsy and a series of genetic tests before transferred into the uterus.

New technologies are now allowing some couples to choose the sex of their child before pregnancy. Many couples wish to have a child of a particular sex in order to prevent certain medical illness or to help balance their families and this is known as gender (sex) selection IVF. North Cyprus Fertility Clinic is the only clinic in Cyprus with its own genetics department, which allows performing gender selection IVF treatment in house.

PGD is one of the most successful methods when it comes to gender selection. This is a complex procedure that involves the creation of embryos that are then analyzed for cell structure and DNA. Embryos are created in a laboratory and allowed to divide. After three or four days, one cell from each dividing embryo is removed and analyzed for DNA and genetic material. Once the sex of the embryos are determined, only those embryos of the desired sex are implanted into the mother’s uterus through IVF. PGD is highly successful, giving you a 99% chance of having a child of the desired gender.

Gender Selection IVF works very much like the IVF procedures explained in the “IVF” section under “Our Services”. However, the embryos that are formed using the IVF procedures undergo genetic testing known as PGD before they are transferred into the uterus. Therefore, in gender selection IVF, the five step IVF procedure requires a sixth step, which comes right before the embryo transfer step.

PGD testing for Genetic Disorders










PGD (Pre-implantation Genetic Diagnosis) technology improves the likelihood of a successful pregnancy and birth for two distinctly different groups of patients. Couples with infertility related to recurrent miscarriage or unsuccessful IVF cycles and couples who are at risk for passing on inherited genetic disease to their offspring.

PGD genetic diagnosis is performed as a part of an In Vitro Fertilization cycle where multiple eggs are produced, retrieved from the ovaries and fertilized with the husband's sperm in the Embryology Laboratory. IVF is necessary to give us access to the embryo in vitro. At their earliest stage of development, one or two cells are removed from each embryo through a procedure called embryo biopsy. These cells are analyzed in the PGD Laboratory to determine which embryos are free of genetic abnormalities. This sophisticated and technologically advanced testing identifies which embryos are free of abnormalities and more able to achieve the patient's goal of a healthy baby. PGD can screen for genetic disorders in chromosomes X, Y, 13, 18 and 21, which make up for about 85% of all major genetic conditions such as Down's syndrome, Tay Sachs and klinefelter's syndrome and many more.

PGD tetsing is also an option when patients require screening for a specific genetic disorder. When the gene that carries the genetic disorder is identified, single gene analysis can be performed in order to screen for the specific disorder. A special PGD probe can be built for this purpose as long as the specific cause of the genetic condition has been identified.

Preliminary Work


Obtaining Medication:
In order to obtain your medication, you can either contact your local gynecologist and kindly ask them to write you an order so that you can purchase your medication locally at your pharmacy, or, you can use the Fazeley pharmacy located in the UK.
The Fazeley Pharmacy is an international pharmacy and they can deliver worldwide. However, if you are not located in the UK, you will need to allow for at least 12 days from the day of your order until the medication arrives. The pharmacy also charges a fee of 25 STG to transcribe our prescriptions into a UK prescription. Ms. Bushra is the director and she will gladly help you sort out your medication needs.

Vitamins:
Vitamins are an essential part of your treatment. It has been shown that folic acid supplements not only help you maintain a healthy pregnanyc, but also help with your baby’s neural development, therefore, it is important to start using folic acid prior to embryo transfer and continue using for the duration of your pregnancy.
Similarly, use of Vitamin C tablets (precisely, 500 mg) has been identified with increased quality of eggs retrieved, therefore, it will be important to use vitamin C supplements daily for at least 2-3 weeks prior to egg retrieval.
Finally, because your daily food intake may not provide you with all the essential nutrients, it will be important to use a prenatal vitamin complex prior to and throughout your pregnancy. Pregnacare or any other one-a-day prenatal vitamin complex will be useful.

Using Medication:
Some of the medication you will use in your protocol are in tablet forms, so they are used orally. However, some of the medication are in injection form, therefore, it can be a bit more complicated to administer them. Some patients find it useful to have the injections administered by a nurse or a doctor first so they can learn how to do it and they can do it themselves afterwords. Even though the specific time of the injections does not make much of a difference with respect to outcomes, the time elapsed between each injection does. Therefore, it is important to be consistent with your injection timings. Make sure that there is approximately 24 hours between each injection. For instance, if you administer your Gonal-F at 8 AM in the morning the first day, make sure you administer it around the same time the next day. A couple of hours of difference is acceptable.

Where to get your scans?
Since majority of our patients come from abroad, and given that in our hectic lives, we all have work/family or other commitments, not everyone is able to travel for treatment for a long period of time. In order to shorten the length of your stay without compromising your chances of success, our protocols are prepared so that you can have access to your medication and follow a certain treatment calendar prior to your arrival in Cyprus. This minimizes your stay down to 8 days. However, in the meanwhile, while you are using your medication, you will need to have a couple of scans in order to observe how you are responding to the medication and whether we need any dose adjustments or additional supplements. For this end, you can either see your own gynecologist for the scans if they are willing to cooperate, or use one of the locations we recommend.
1- For UK patients, Cherish UK is a great place to have hormone testing and other blood work as well as scans. Alternatively, the Birth Company is also able to provide our patients with their preliminary tests and scans. 
2- For US patients, we have a number of locations that we can recommend. However, since the US has a very large geaography, our locations may need air travel, which may not be convenient to all patients. Please ask Ahmet directly about the US locations.
3- For Turkey patients, there is a location almost in every city where we can recommend for your scans, please ask Ahmet about these locations.
4- For patients from the Netherlands, we have a local representative who will be in charge of all the preliminary work, Mr. Bert Van Delen. We will get you in touch with him once you are admitted into our treatment program.
5- For other locations, please ask us if we have any recommendations. We have options for patients from Germany, Switzerland, Italy and France, but only in the capitals.

Traveling to Cyprus:
Cyprus is a country where the weather is usually warm during winter and hot during summer. It will be in lower 10s during winter and upper 30s during summer, so make sure you pack your clothes according to the weather.
There are two routes of travel to Cyprus. One is arriving at the Ercan Airport in the North, which is about 10 minutes drive from our clinic. The airport does not have directl air links, therefore, there will be a touch down in Turkey for a brief period. You may want to check out flypgs.com or turkishairlines.com for pricing information.
The second alternative is to travel to Larnaca or Paphpos Airports in the South. This would necessitate a border crossing and approximately ninety minutes of travel time to reach the North. If there aren’t huge savings with this option, we often recommend the Ercan Airport due to its convenience.
The electric plugs in Cyprus are similar to the ones in the UK, however, patients traveling from other countries should bring an adaptor with them if they are planning on bringing electrical equipment with them (blow dryers, cell phone chargers).
Do not use your cell phone excessively unless you have to. The roaming charges will be excessive and you will face a huge bill upon your return, therefore, it will be wise to obtain a prepaid phone chip with one of the local carriers to use in your cell phone to avoid those charges.
If you are staying at the Golden Tulip Hotel, it is very conveniently located right next to the clinic. You will find plenty of dining and shopping alternatives within walking distance. If staying at the guest house, you will still have access to supermarkets, and a few dining places. The guest house is about 10 minutes driving distance from the clinic, but our driver will be able to take you back and forth for your appoitments.
